Captain Acknowledgment — Required
Complete the Captain Acknowledgment Form by first game day.

Match Report
After each game, review your team’s final score, goalscorers, jersey numbers, and red cards. If any information is missing or incorrect, submit a correction by Wednesday at 8:00 PM following the game.

Red Card Appeal Form
If you strongly believe a red card issued to one of your players was incorrect or warrants further review, you may submit an appeal of the resulting one-game suspension. Appeals must be submitted within 24 hours of the game.
